SCHOOL, SPORTS, & CAMP PHYSICALS
School, Sports, & Camp Physicals
Before your child starts school, joins a sports team, or goes to camp, it’s crucial to ensure their health and readiness. UHM offers comprehensive physical exams for students of all ages. With walk-in availability and extended hours, we provide the convenience of urgent care along with the care you’d expect from a primary care provider. Our school, sports, and camp physicals are affordably priced.
Please note that this service is an out-of-pocket expense, as it is not covered by insurance. It includes one form for school, sports, or camp physicals.
Preparing for Your Child's Physical
Please make sure to bring any forms provided by the school, sports team, or camp as well as the vaccination records for kids under 6 years of age and any other relevant blood test results (e.g., lead and hemoglobin). This helps ensure efficient and high-quality care for your child.

What to Expect During the Physical Exam
We care about your child’s health. During a physical exam, we check their overall well-being and look for any early signs of illness. We also offer extra tests for lead, anemia, and tuberculosis (which may have additional costs).
Our medical team tailors the exam based on your child’s age, gender, and the reason for the visit. A typical physical includes:
-
Gathering family and personal medical history.
-
Measurements of height and weight.
-
Evaluation of flexibility, pulse, posture, joints, and more.
-
Vital sign measurements, such as blood pressure, body temperature, and heart rate.
-
Assessment of body systems, including the lungs, heart, eyes, nose, and ears.
Our medical provider may also ask about alcohol and tobacco consumption, dietary supplement usage, recent immunizations, and sleep habits. For children under 18, parents or legal guardians must be present in the examination room.
